sana may makasagot sa tanong mo...

Hi msmckulit. In my case additional docs requested were the following: personal history for a certain period; transcript of records and diploma for highest education attained; employment records. All documents requested were for me and my wife.

hi poxy, I had the same situation as yours, wrong province but their letter reached me cuz our Post Office sorts letters thru zip code which is fortunately correct but you should inform them cause, the wrong Address will reflect on all your coming forms like Medical request. Click the link below the address to inform them..hope this helps.

Hi.there Rak. No need for notarized copies. The check list did not say naman that CIO needs notarized copies. Some of my docs were notarized not really intended for CIO but for MPNP sana. My other docs were NOT notarized naman.
