they should have just say indicate middle name. but if I'll follow the passport ID it should be:

will it also applies with my parents information.
I suggest you read through the guide first - Application for Permanent Residence – Provincial Nominee Program (IMM P7000)
Many applicants go through the forms and in the excitement in filling them up, focus on the physical act of putting in their information on the forms and check-them off against the available checklist - thus skipping the guide which is inadvertently located last in the list for forms. It has a lot of information that helps in filling out the application.
The guide should be your first doc to read, and your generic instructions. Specific instructions are mostly specified in the forms. So Generic Instructions (i.e. Guide) + Specific Instructions (i.e. inside the forms) should be enough to clarify a lot of the questions in filling up the forms. You should be always going back and forth between instructions on the guide and the specific instructions in the forms.
Example: In the guide it is stated in filling up the Principal Applicant's Personal Details:
Indicate all of your given name(s) (first, second or more) as they appear on your passport, travel or identity document. Do not use initials.
- I have highlighted the things here which indicate you should be adding in your middle name. Middle name falls under "or more". The phrase "as they appear on your passport" clearly guides you to use the names as seen on your official identity document. PH passports include/show the middle name. "Do not use initials" identifies the common mode of showing a middle name as a middle initial (i.e. Juan P. Dela Cruz) is not recommended, so spell out all names.
In the form IMM5669E.pdf, #1 says "Your
full name" - in the PH your full name is composed of
Family Name + Middle Name (MI) + Given Name(s). The spare box allows you to put in your other names (including your middle name) to match that of your passport/national identity as much as possible.
Canada is one of the countries that commonly does not use a middle name, middle intial, or the mother's maiden name as the middle name. In my travels, I have been asked also in Australia - but this is due to the names shown on the visa does not match that on my passport. The main guide in filling forms should be - it should match that of the passport to eliminate ambiguity.
- Juan Marcelo Dela Cruz is not the same person as Juan M. Dela Cruz based on names at first inspection - so questions arise to the identity.
- Similarly Juan Marcelo Dela Cruz is not the same person as Juan Dela Cruz based on names at first inspection and will again trigger questions on identity.
And lastly - be consistent in filling out Personal Details....
