here are some things you might want to or have to think about.
Since they've asked for a passport within a month of getting her app in ND, it will take few months. my guess june or july she will get her visa (counting the time ND replies to you with your enquiry and then you sending in the passport and they don't need anything else)
with that said, since your wife is on TR visa she is not covered by any health care so if she gives a birth before her visa comes (or it's a preemie) you will need to pay for cost of delivering her baby usually costs around $4000 to $20000 if its straight forward pregnancy, but you stated your doctor advised not to travel than to me that doesn't sound straight forward so it could be more costly. So look into your employer insurance if you have one, double check with your provincial insurance or look at some private insurance which could be difficult to get now. Other option to save money is find a midwife but if its not straight for pregnancy than probably not a wise idea.
Secondly, CIC might ask for medical certificate advising your wife not to travel as pregnant women can travel by air up to week 36 in a straight forward pregnancy.
Just out of curiosity, your wife was on TR visa so how come for PR you didn't use the application process for applying within Canada? It takes a little longer but she could stay in the country even after her TR expires until her PR decision is made. On a side note if your wife gets called in for an interview after she sends her passport, then they can make arrangements for her to go to one of the offices here in Canada for an interview, most likely in Vegreville.
