Hello,

I am filling the forms for my 4 months old son. Me and my wife both have PR I have a few questions:

1) What do I fill in the form 5669 for the sections Education History and Personal history as my son is just 4 months.
2) for the form 5481e in the section Number of Family members and persons included in undertakings in effect and not yet in effect for the Bi what is the number that has to be shown. (Number of person included in previous undertakings and not in Effect.

Please help me answer the questions. Thanks in advance.

5569 - Educational History - I have mentioned 0 in the 4 boxes and N/A in the level of education section.
5569 - Personal History - I have mentioned activity type = other; job & company fields - Not Applicable
5481 - Did you previously sponsor anyone for PR?

Thanks a lot for the reply. Yes I sponsored for my wife last year.

then as far as I know, you have to enter that info in the previous undertakings section. I will let someone comment on this as well.

I have two more questions.

1) Can I be the Sponsor and the Guardian as well? I am in Canada but my wife and child are in India.
2) for the form 5604 E who fills it out my wife who is in India and gets notary done or do I fill it?

Everything is filled by you. Please add a Letter of Explanation and a letter from your spouse that she is fine with you sponsoring the child.

you don't have to fill 5604, just get a letter signed by your wife saying that she is fine with you sponsoring the child and she will accompany the child to canada.

"We sent you medical instructions on March 11, 2020. To avoid delays, please provide us the information requested in the letter as soon as possible. Please consider delays in mail delivery before contacting us. "

Should I expect a physical letter to the address in India or an email? I haven't received any email for the medical yet.

try different Caps , no-Caps combination , as i remembered that's how it worked.

Hi... I’m sponsoring my 15 y/o son. Do I need to submit both elementary and High school records or one of them is fine? I don’t have the elementary. But I do have the high school records only. My question is.. the immigration asking for both school records? Pls help me. I can’t provide the elementary school for some reasons.
