(Please declare personal history and addresses since the age of 18 without any gaps in dates)
1. New Application for Permanent Residence IMM0008 form for the principal applicant.
2. Schedule A – Background/Declaration [IMM 5669]
3. Supplementary Information - Your travels [IMM 5562]
4. New Additional Family Information Form IMM5406 for yourself and each family member over the age of 18.
5. Copy of passport title page for each applicant. If any of your dependants are included in a parent's passport, please ensure that you apply for an individual passport for each person. Please note that each person will require their own passport at the time of visa issuance.
6. For all applicants with any military service, please provide a copy of the Military Card or Military Exemption Card.

it usually is attached with the email...........if u didnt get it send CSI to PVO for this form................
or any forum member who got this form can send u................

Silly question but my letter says: "Remember to quote your file number on all correspondence, including the envelope". Do I have to put my file no on all new forms that I am filling out and sending as requested by PVO?

Secondly the letter also mentions "(Please declare personal history and addresses since the age of 18 without any gaps in dates)". However, all new forms mention "Provide details of your personal history since the age of 18, or the past 10 years, whichever comes first. Start with the most recent information.". Do I follow the form or the letter?

Silly question but my letter says: "Remember to quote your file number on all correspondence, including the envelope". Do I have to put my file no on all new forms that I am filling out and sending as requested by PVO?

1st no question is silly....everybody is learning here on daily basis.....this mean just QUOTE UR FILE NUMBER on ENVELOP in which u r dispatching ur documents.....so that at reception they differentiate which envelop is for whome VO based on FILE number written on envelop

Bro I don't think there's a separate form for Military service. All Military service questions are covered in form IMM5669E.

Point no 6 in PVO letter requests you to provide copy of the Military Card or Military Exemption Card.
