service. For all applicants with any military service, please provide a copy of the Military Card or Military Exemption
Card.

or just what you mentioned in your post

If first statement is true then you do have to send military form otherwise you may choose not to send.

5. Copy of passport title page for each applicant. If any of your dependants are included in a parent's passport, please ensure that you apply for an individual passport for each person. Please note that each person will require their own passport at the time of visa issuance.
6. For all applicants with any military service, please provide a copy of the Military Card or Military Exemption Card.

Regarding "PLUS INFORMATION & DOCUMENTS REQUIRED (by mail) , you can see below this will

X - PCC for applicant and spouse - country name

X - RPRF fees payment

You need to attached PCC + demand draft for RPRF fees

You have enough time and just need to update the forms. No other documents required to be sent other than the list mentioned by them

Just to share one info. after validating Generic form on the last page there is a note " Do not forget to include: photos, fees (if applicable), and any other documents required by the visa office.....".
Then why we are not sending photos?

Hi yukhan,

In generic form IM008, you need enter details for urself, spouse and dependants

After validation, it will generate bar codes which will help them in scanning your data thru scanner

I think otherwise.. In LVO letter, it says to complete Military Questionnair which was attached in the email. However, you dont see this line in the letter from PVO. It just asks to provide proof if you have done the military service.

i think it's not mandatory but recommended that's why i said you may choose to.
