So, I am confused.

From where shall I surrender citizenship? Options
1. From https://indiancitizenshiponline.nic.in/Renounce/
2. From https://portal3.passportindia.gov.in/Online/index.html

Any help from someone who has applied already will be great.

Note:
1. I am not submitting request to get "surrender certificate" - but trying to renounce citizenship
2. I am not applying for OCI

In the checklist (link -> document requirements -> click here), it says "(b) Online Application Form " - that's my question originally - where do I get that from.

Please suggest.

I am not sure if I understand your question here - you want to give up your Indian citizenship? When you renounce your Indian citizenship it means you will get a surrender certificate.

The checklist says to paste the picture on the paper application. By paper I mean the Print Out of online filled in Application

Yes there is box where you have to fix your picture.

I require assistance. I am attempting to complete the surrender certificate application for my two girls, who are 10 and 17 years old. However, when i reach Family details, I do enter parent details . I would appreciate your help and guidance on how to proceed with this issue. After clicking save, I then get an error asking to Enter spouses details. Is anyone else experiencing a similar problem?

You should be selecting "Marital Status" as Single.

I Did select as single. I called BLS they said create a new application could be a system issue. I created a new application and had the same issue.

The spouse columns gets generated only if I select Married. Otherwise it works without any issue.
Try with a different device.
