Supplement to Background Declaration

In the immigration guide there is a form called Suplement to Background Declaration. It asks 2 questions. Do we send this in with the application? Where does it go with the app if so?

Put it behind the Schedule A (Background Declaration).

Thank you! I will do that! Now there was another form in the immigration guide labelled Declaration from non accompanying parent ...now that form does not apply to us...would I include it and just write on it N/A or just not include it at all. And also for all the other firms in the app....if none apply should I include them still and write N/A or just not include??? Thanks!
