After having my examination, the hospital gave me a sheet

Barcode Number on Top of the Sheet: UMI U##########

My Photo in Black and White on top as well

Personal Details as well -

IME: Non EDE
Upfront Medical Type: Worker

Health Case Status
This health case has not yet been submitted to IRCC. The status of the individual examinations is listed below.

501 Medical Examination - Required
502 Chest X-Ray Examination - Required
707 HIV test - Referred - Clinic Name
712 Syphilis Test (VDRL or RPR) - Referred - Clinic Name

Consent Provided
On <Date> you consented online to using eMedical to process your health examination where available.


1. Am I supposed to just scan this document and upload it in my eAPR?
2. My thinking is the hospital will be the one to submit my results to IRCC?
3. Will I know my medical results as well?

Down to few more days before my application submission so I am exhausting everything that I need to clarify

1) yes u have to upload that
2)IRCC will itself do correspondence to the clinic/hospital
3) yes u can contact that clinic to release your results to u

and another thing, is this the only document that I need to upload on the medical section of the application, or is there anything else
