Study permit queries: Have you previously applied to enter or remain in Canada?

I am a Bangladeshi citizen. I have got admitted to a Master program at the University of British Columbia and have also got the letter of acceptance. Now I am applying for my study permit.

My query is related to the form IMM 1294 (Application for a Study Permit Made Outside of Canada). In this form, box 2 letter C of “Background Information” asks a question “c) Have you previously applied to enter or remain in Canada?” Last year, my husband applied for FSWP 2014, but his application was returned as his documents reached them after the quota was met. In his application, it was mentioned that I would accompany him to Canada. Therefore, I am confused about the answer of this question “c) Have you previously applied to enter or remain in Canada?” as I was not the principal applicant.

My husband will also apply for TRV to accompany me. It would be helpful if you can inform me how he should answer this same question in his form.

Furthermore, my husband has opened an Express Entry profile. Could you please tell me if this profile can create any problem in the processing of his TRV and my study permit?

You must disclose it since you are 'accompanying dependent' on application. Otherwise, 5 years ban for misrepresentation.

Many thanks! If I answer "yes" to question "C) Have you previously applied to enter or remain in Canada?”, how should I explain it? As my husband's application was returned before being processed, he does not have any file no.

Could you please tell me what should be my replies to "B)have you ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?"

Furthermore, my husband has opened an Express Entry profile. It would be helpful if you can tell me whether this profile can create any problem in the processing of his TRV and my study permit?
