She was approved while outside of Canada. Does this question mean the first immigration office she arrived at in Canada, like I think it was Toronto where she landed.
When she was approved and got the visa is irrelevant. It is possible to be approved and get a visa but never land and hence never become a PR at all. Therefore, the place they are talking about is Toronto if that is where she arrived and filled out her PR paperwork. It should also say that on her PR card.
