Status not changed after IMM 5475

Hi All,

On September 4th, I received a notification asking for IMM5475 form. I filled and uploaded the same the very next day. it's been about 3 weeks but status of my application is still "Awaiting your Action", is anyone else facing the same issue?

Check whether you submitted/transmitted it....you will be taken to a page where you would be asked to sign then it’s officially transmitted. You

thank you for the response but i am not following you. they message they sent was a plane letter in message inbox which asks me to send them the form. I found the form online and filled it..signed it and uploaded. As of now, under the section "Document submitted by client" i see an entry of the form that i uploaded. i dont know what to do with "you will be taken to a page where you would be asked to sign then it’s officially transmitted". please elaborate.

Yea, when you upload, you need to go back to the document section... I’m sure it will say “uploaded-not transmitted” in regards to the doc you uploaded. It means you have not officially transmitted it.

You need to click Continue button on the bottom left I believe which takes you to a sign page (you write your name) then you transmit. Did you go through this process?

oh yeah, I did go through the process, thanks for clarifying. even after that there hasn't been a status change. is there any way to clarify this as this came with a deadline of 7 days. even though i uploaded the form within 24 hours but still i am a bit worried as status hasn't changed. thank you

yeah, i did that as well about 6 days back. even though the form says it takes 2-5 days but immediately after i submitted the form it said that they will respond back within 28 days..

Guys - While I know about second point, what is the first point? It is just a number with my name against it. And they have given me only a weeks time to submit. Can anyone help me here?

The numbers you see before your name is your UCI. You are being asked to provide just one doc.

You just need to fill the form in below link, scan and sign it and upload it under your documents:

It won't take more than a few mins for you to fill and submit the form, after submitting the form it took them about 4 weeks to update my application status from 'action required' to 'In Process'..
