I'm still confuse on what date to put in the "to date" box....

a). schedule a background/declaration form

> question 8 on my personal history. I'm still working with my current employer from 2011 to present. so what should i put in the "to date box"?
can i just put the expiation of my visa which is on 2015?
> question 12 addresses: is it okay to consume 2 boxes for my address? because my complete address in philippines is not enough for one box
only.

b.) country specific form (this question applies to those who's visa office is in ABUDHABI) but if you have any idea help too Smiley)

> in supplementary form/travel history: i went back here in dubai 19/01/2013 till present so what should i put in the "to box"? i'm thinking to put
till date or till present. is this okay?
> in residency questionnaire:question 2- my husband has been physically present in canada from sept. 2010 till present so should we put any
dates in the "to date" or just put also till present? (example: 09/2010 to present/till date)
> in residency questionnaire: question 8- will me as the principal applicant cancel my gulf residence visa? if yes will be my answer i will put the
date of expiration of my visa. if no is my answer i will just put the reason is because i will wait for my approval or pr then i will cancel my
visa. is this correct? im not so sure what to answer.....

c.) in the document checklist being as the sponsored person can i put "x" on the box if it's not necessary for me to include the documents? or just leave it blank? and should the document checklist be at the front page/top page then followed by my forms and additional documents? or the barcode first in the generic application?

d.) i'll just need to provide 8 photos in total and put my name, date of birth and when the picture was taken right?

I might not be able to give you the best answers. My responses will be merely my best guesses.

for schedule a question 8 in 'to date box' don't put ur expiration date of ur passport, instead simply put 'present'.

yes it should be ok to consume two boxes to write ur address.

b)

- in 'to box' write 'present'
- again, just say 'present' in 'to box'
- i am actually about to fill that too and i am thinking of saying 'yes' i am going to cancel my gulf visa but also add a caveat saying as soon as i get my CA PR. no should'nt be your answer unless the place where you are working restricts you to cancel the visa or has some sort of contractual clause that forbids you from quitting.

c)

i am not sure what you should be doing on this one. for me i don't think i am gonna do this because in their recent request they did not ask for the checklist...

You can just put present on the following any how they will ask visa cancellation.

yes its ok.If the space is not enough just make a table in the excel and you can write there...

Till to date

You can write the date of signatures that will represent till to date

Just write Cancellation of the Gulf residence will be shown as soon as get approval for my PR application

Leave it blank will be ok you can choose what you have included...

Just need to provide photos no where describes to put the name date and signatures as this process is done while applying for passport application not the visa application.

Hi the present date just put the date u fill up the form.and its OK to have another copy of form for address,and just leave blank to the document checklist that u can't provide.

hi scoobydoo... thanks for the link... i saw this link before but the link is broken... it gives an error message as soon as u get in there... i am soo mad right now... not even funny... they ask the very form that is not available online anywhere... like ANYWHERE... it feels as if its a big conspiracy to stop people from going to their families regardless of how genuine it is... and then they have go for a strike to get a pay raise...

I think the supplementary form you need is at the country specific form. you will there in appendix a the supplementary forms like TRAVEL HISTORY, EDUCATION, MILITARY SERVICE ETC. just check in cic website then find forms then you will see it in the section right after the sponsored person forms...

i have got it guys.. you guys have no idea what i have gone through to obtain this form... anyhow.. i have saved it as pdf format on my pc... i want to upload this forms so that people can use it... this will be of great help.. i just don't know how... any suggestions?
