thanks @ vanity08. dun ka pa ba sa IOM Makati? parang meron din po ang st.lukes. 5250 ata fee.. yung medical form po sa clinic /hospital na fill out?

I had my re-med at IOM in makati,so ung form ko for re-med sinend lang ng CEM. U can also try st.lukes, don't know the fee eh.
