It is a sponsorship for spouse outside canada, the destination is in Winnipeg Manitoba however I forgot the date of his medical examination...

oh the date of his medical was last March 7, 2013.. and there are no dependents and accompaniments. Thank you!

almost 2 months na hindi parin domating yong passport ko na e send ko june/07/2013 hangang ngayon wala pa.. sana pag pasuk nitong buwan nag august sana may resulta na... godbless..

Yun Appendix A filling up is part of the passport request step (PPR). Sinusulat mo lang dito yun family information mo like name, date of birth,passport number and expiry date. The info needed is for your whole immediate family whether kasama sila sa sponsorship or not but sa mga kasama lang required yun passport number and expiry date. Kelangan din dito yun contact number and address kung saan nila ibabalik yun passport mo. Sa PPR letter maari humingi sila ng additional requirements or documents and it depends po from case to case kung ano po ang hihingin nila.

WOw!!!! Congrats sis!!! ;D Kinuha ko yung track # nang package ng application namin, sabi sa Canada post expected delivery July 31 to Mississauga, ngayon kaka-check ko lang sabi dun "item processed at postal facility 12:34 Mississauga"... Di pa natanggap noh!!! :'(
