Regarding to get an original Receipt IMM5401. Sabi 2 ways to get that, #1 is to pay online and #2 is to order that receipt at CIC Call Center 1-888-242-2100, and bring it to the bank to pay the application fee. Now, I let my husband called that call center #, he said it was only citizenship according to the telephone operator, no topic about that Receipt IMM 5401... Please help how can my husband in Fox Creek, Alberta acquire that receipt, by Monday I am sending to him our application na... ???

If you want to pay at the bank you need to order the form. Apparently they don't have it for download. The link is:

Hello everyone, someone from Canadian Embassy - Manila called me up this morning and asked about some details regarding my travel and also she told me that they are aware that my medicals will expire on March 29. She also asked me if I am ready to travel before my medical expires. She confirmed my email address and told me that she needs additional information (Personal History) from me and she will send it thru my email within the day.

I got the email after a couple of minutes and to my surprise, they are also asking for my PASSPORT, Appendix A and Photos! After months and months of waiting, finally, I got my PPR already. Thank you, LORD!

I hope the visa will come out very quick, though.

hi gaia, just asking in what form did you put your personal history as additional document need by the v.o...is there any form that they sent to you? coz they also need my personal history.

Kami naman sis nung Feb 20 nagparemed, and same with you I was also asked to undergo Hepa B and Hepa C screening because sa exposure ko naman for 1 year sa hospital laboratory from 2004-2005 during my internship as a Medical Technology student. It was good that i was able to read your post because I kind of forgot to ask when the result of the Hepa screening will be released and sana nga they would pass our remeds asap. Me and my 2 kids are from January 2012 batch.

Hi sis nauna lang pala kayo sakin nang one day 3days nga daw ang result but yung mga bagong nag pa remed sa st.lukes 5 to 10 days daw iforward sa CEM yung sakin kc pahabol lang nag pa xray ako feb 7 then nag email ang ottawa para sa medical namin mag ina kaya yun 2x kming bumalik sa st.lukes sabi after nga nang result pag ok na ifoforward na kagad nila sa CEM.... Mag hintay nalang tayo sis sana mas mabilis pero sa mga nabasa ko dito after nang remed 2months then DM di bale after naman ma DM days nalang VISA na... ;D

Hi ane_gwane, ganyan din ang hiningi sa akin ng CEM, sabihin mo lahat ng nangyari sa inyo ng ex mo,kung ano dahilan bakit kayo naghiwalay.pwede mo i hand writen o iprint.lahat sabihin mo para kang gumawa ng history,tapos pina notaryo ko pa.Ganon din si hubby mo magpagawa ka sa kanya ng history ng ex nya kung ano nangyari sa relationship nila kung kailang nagsimula at natapos at mga reasons.Pipirmahan nya yun at ipapadala sayo.kailngan ipadala nya original.

I am ordering the receipt Imm 5401 online, now I am confused? :'( Theres a question Order Info: Number required of FEES RECEIPT IMM5401, and select 1 or 2 or 3. Now which one should I select? Is the Receipt in 3 pages or only one page and means I should select 3 to make 3 receipts? Please correct me!!! ??? I know the other page is for the copy of the sponsor, 2nd is for the CIC then 3rd for the bank...
