hi jury,,una mong gawin gather mo lahat ng evidence proof of relationship nyo muna kasama na pictures nyo sa kasal.after ng kasal hingi ka kaagad ng endorsement ng marriage contract nyo sa Lcr dalhin mo sa nso,gather mo lahat ng original documents,like passport na surname na ng asawa mo gamit mo,marriage contract kahit di muna authenticated wait mo pa kasi muna mapasok sa record ng nso marriage contract so pwede na yan mahuli once they request for ppr mag ask yan sila ng authenticated mc at advisory on marriage,birth certificate authenticated ng nso,nbi clearance with your maiden name.tapos ung application form,tapos padala mo na sa asawa mo,except your passport padala ka lang muna ng photo copy.then id picture nasa site nila kng anong size ang required... then pa medical kana using ur married surname bring your passport,passport size pictures and appendix c form.heres the link where u can download the form

Saka pala, you don't need to change your name sa surname nya. You can use your maiden name para masmabilis. Kasi if you really want to submit your application asap, just use your present passport.

About sa marriage certificate, after ng kasal nyo, Marriage certificate from the local civil registry ang hawak nyo palang. Kaya kung gusto moh makakuha ng NSO-copy of marriage certificate agad, mag ask kana lang sa local civil registry ng advance processing sa cityhall para ma-forward agad sa NSO yung local civil registered marriage certificate nyo. Kasi NSO copy ang required plus Advisory of Marriage from NSO.

For medical exam pala eto requirements:
You must bring the following items with you to the doctor:

Appendix C for you and each family member who needs to undergo a medical examination;
passports for yourself and your dependent children, including, for each, a photocopy of the bio-data page (page in the passport which contains name, date of birth, country of origin, etc);
eye glasses or contact lenses, if you or your dependent children wear any;
previous medical reports, if any;
recent photos of yourself and your dependent children (See Part 3: Country Specific Instructions).
The Appendix C is provided in Part 3: Country Specific Instructions.

You and each of your family members must complete your own copy of the form. Make enough photocopies of this page for your needs before you start filling the boxes. Make sure your family names and given names are spelled the same way they appear in your passports.

Important information. If members of your family are Canadian citizens or permanent residents, they do not need to pass a medical examination.

Authorized doctors

Your own doctor cannot do the medical examination. You must see a physician on Canada's list of Panel Physicians.

Note: The physician is only responsible for conducting a medical examination; he or she cannot give you any advice on the immigration process.

Exam validity

The medical examination results are valid for 12 months from the date of the medical examination. If you are not admitted as a permanent resident during this time, you may have to undergo another medical examination.

Medical fees

You are responsible for paying all costs related to the medical examination.

Note: You will need to include proof (Copy 2 of the Medical Report: Section A, provided by the panel physician) that you completed the medical examination.

Kami ni hubby ko nun first na ginawa namin kumuha kami ng :

-LEGAL CAPACITY nya sa CEM singleness yun na katunayan na di pa sya nakakasal sa Canada
-CENOMAR sa kin nag order ako online sa NSO

After wedding ako na mismo nagforward sa NSO ng marriage contract namin 3 copies kasi yun yung pinaka original nasa kin yung isa na kay hubby yung isa naka attach yun sa ibibigay na Municipal sa NSO at sinabi sa kin na sa LBC courier ko ipadala nagbayad ako sa LBC then yung receipt ipinaprint at inattach dun sa request ng marriage contract.

Then sa receipt may nakalagay dun na date kung kelan pick upin sa NSO di pwede delivery kasi daw first time so nung pinick up ko sa NSO konti lang pila sa kuhanan ng Marriage Certificate 2 copies ang nirequest ko nun naka attach na dun sa kin yung AOM Advisory on Marriage. Akala ko nga kung ano yun muntik ko pang tanggalin nung ipadala ko kay hubby kasama ng mga Application form and other Documents .

Tapos sagutan nyo na para incase na matagalan pa hubby mo dito sa Pinas mas safe din para matulungan ka nya sa pagsagot. Ako kasi nun ako lang nagsagot tapos chineck lang ng hubby ko kung ano mga isasagot ko sa skype.

Tapos kumuha na rin ako ng NBI ginamit ko na ang last name ng hubby ko.

May 26, 2011- Nagpamedical ako sa SCTS Makati tapos binigyan ako ng Copy 2 ipaprint ko daw yun at isend ang original kay hubby photo copy sa kin
May 28, 2011 - Umattend ako ng Counseling for CFO para magkaron ng certificate para mapalitan ko ang status ko sa Passport requirements kasi ng DFA kelangan yung ipaphoto copy yung certificate galing CFO sa Q.C. Pag may visa na ko babalik ulit ako sa CFO para idikit na nila yung sticker sa Passport ko .

May 29, 2011 - Passport Appointment kasi papalitan ko ang passport ko green to maroon (machine readable) saka para mapalitan na rin ang status ko from single to married .
June 2011 - nagpunta ako ng DFA para sa appointment ko
July 16,2011- Delivered my new passport married status na at gamit ko na ang last name ni hubby


Tapos sinagutan ko na lahat tapos pinadala ko lahat sa DHL papunta kay hubby sa Canada ang mga ito:

DOCUMENTS- nbi, legal capacity ni hubby, marriage certificate & birth certificate & cenomar, AOM advisory on Marriage authenticated from NSO,

RECEIPTS - sa western union, hotels, resorts, plane tickets, restaurants, cinema, telephone bills lahat ng resibo nung magkasama kayo.

PICTURES- nung mag bf gf palang , pag namamasyal , nung wedding, kasama ang family, lahat .

EMAILS- lahat ng emails lalo na yung importante nung nagpaplano na kaming magpakasal pinirint ni hubby

COVERSATIONS- pinirint din nya lahat ng conversation namin sa skype, ym, msn.

ADDITIONAL - birthday card, valentine, xmas, monthsary lahat ng cards, souvenirs like welcome note sa resort goodnight note

ADDITIONAL- essay or kwento ng love story namin mula umpisa kung pano kami nagkakilala mga ganun sya ang gumawa nun si hubby

Kami di na namin yan sinagutan or pinirint or pinasa or sinali kasi di kami kumuha ng Representative, kung kaya nyo naman gawin ng sponsor mo kayo nalang gumawa at mag ayos ng mga Application mas mura pa. Ganun din naman yun kahit meron or walang Representative case to case basis walang pinapaboran. Magsasayang ka lang pera ganun din maghihintay ka rin tulad namin walang Representative.

Petergriffin thanks a lot. But I have so many questions in my mind po that need to be answer so we can go on. First,the one that you mentioned about the "LCR" thing,ano po ba yan?saan yan?

= I have the passport na but hindi pa surname ng maging asawa ko, I have to change my surname right?HOw long does it take?and kailangan ng marriage cert for that di ba?

= yung sa marriage certificate, di ba matagal yun ma authenticate sa NSO? saan kami kukuha noong sa hindi pa authenticated?ano sasabihin namin kung magpapasa kami na hindi pa authenticated yung sa marriage cert namin?

= yung sa nbi clearance at medical exam since na kailangan surname ng hubby ko,pwede ko po ba e show muna yung hindi authenticated na marriage cert kung kukuha ng nbi clearance? saka yung sa passport,pwede po ba na yung hindi muna authenticated na marriage certificate e so show namin para ma change lang ang surname ko?

hindi mo n kailangan magpalit ng last name kng ano name mo sa passport mo un ang ilagay mo sa lahat ng docs mo oo pwede ka na magpamedical mern din list ng hospital and dr na pwede mo call to appointment mern bbgay na copy sau ssma mo sa application mo .. ung marriage cerificate nakuha ko the next day after ko kinasal!!! and NSO marregister na un sna mapaaga dko lang sure ...kng mern ka kilala sa city hall mas madali dba gooduck!! and make sure mern kau photo copy sa lahat and need d n ng aasawa mo ask for OPTION C ng income nya just to make sure kc 2 weeks dating nun pero thats ok pwede xa request sa gov... .. dobol check mo lhat ung check list mo !! kng mern ka question ask mo lng pwede mo nmn na download sa website lhat ng need mo or ask mo sa asawa mo na call ung gov for documents pero same thing nmn eh pphotocopy mo lng

thanks so much.It helps me a lot. but bothered lang kasi ako 'coz sabi ng iba kailangan ko daw dapat na makalagay s amga documents ko is surname na ng maging husband ko. so nalilito ako. So, "mrsh", is it okay na ba na magpa medical exam using my maiden name?pati na yung sa nbi clearance ko?sakin pa rin hindi nya surname gamit ko?

=almost 30days sya mag stay dito sa pinas,do u think makaya namin lahat gawin from the wedding,kuha ng marriage certificate,nbi clearance,medical exam,kaya kaya namin yan?para madala nya pagbalik niya sa Canada?

pasensya na po if madami ko tanong.. I just want to make sure. sensya na po..and salamat sa lahat ng sumagot..sana pag may mga tanong pa ako sasagutin niyo pa ako..hehe..ang kulit ko..

hello.. ang sa akin naman ginamit ko ung surname ni hubby.. 14 days pwed na daw ma kuha kapag nag advance endorsed ka sa NSO after ng wed nu.. ( mine 3 weeks ko na kuha xa NSO) tpos xa nbi naman one day ma kuha muna un at ung medical 2 days.. at last ko na kinuha ang passport ko na bago. start kna mg collect ng supporting documents at mg fill up ng forms pra pag dating ni hubby mo e finalize nu lng.. ginamit ko ung bago kung fam name ko xa medical, nbi clearance, police clearance, passport, CFO. ako after na ng wed namin nag pa medical 2 days na kuna ko na ang copy.. ask mo lng xa iba kng ok na b4 ng wed mo mg pa medical.. candian ba hubby mo or pr?
