1. As the spouse being sponsored you will not have a UCI yet...Eto po ung nasa guide... "Indicate your Universal Client Identification number (UCI) or Client Identification number (Client ID), if known (8-digit number). Otherwise, leave it blank. 2. It's a must to undergo a medical examination. You don't need any request from the embassy. You just need to download from their website cic.gc.ca ung country specific instruction for the Philippines which is IMM3905. Then bring the Appendix C to your DMP (Designated Medical Practitioner) of your choice. You can find DMP's here http://www.cic.gc.ca/dmp-md/medicalinfo.aspx?CountryID=2009&CountryName=Philippines.
