Hello,

I am PR and I got married reccently from India. I am going to sponsor my wife.

I learnt that my wife has to do the medicals before. But I was wondering if anybody could answer where that has to be sent(as it is requied on form Appendix C )? to CPC Mississauga or the regional office New Delhi?

if you sent already the application in Mississauga, wait until your application have been sent to the visa office in india and send the medical form to the visa office. if you did not send your application to CIC,send the medical report with your application to Mississauga.

Thanks! Are you are sure? I have not sent my application to the Visa office yet! I advised my wife to go to medicals this week. I learnt that after she does the medicals she will get a receipt which I can attach with my application to CPC Mississauga. But when she goes to doctor she has to take filled Appendix C with her. There is column in Appendix C as "Visa Office" what should she write there? CPC M or CPC ND?

my application has been returned to me today june 5 2012..bz i did not submit medical with it....i submitted my application on 28 dec 2012...
they where holding my file for 5 month and told me today that you have to re-file...again all new..with medical...
so dont make mistake of doing it later....

The appendix C in our case we filled out took to our appointment, and then the doctor sent that along with one copy off the receipt he gave us, and the results to Canada. We had to pay the DHL fees. Then, we just included our copy of the receipt with the application and sent it to Mississauga with our application .

there are a couple of versions of the medical forms... when i was filling out the file for my wife (Althoug for pakistan) i too was confused over this part... i did two things... i sent them both to my wife to take to the exam, and the ppl there chose the correct one and discarded of the wrong type... we also called and made sure to which one it was, but still had her take both versions just in case... so fill out both... they will pick the proper one as they do it day in day out...
