I'm getting a little confused about what you are asking. For the FBI certificate, you must have your fingerprints done in order to obtain the certificate. What the form is trying to tell you is that you must tell the FBI why you need them. AND you must also tell the company/agency who is preparing the fingerprint card why you need them. Taking Appendix A of the Region Specific Guide for the United States with you when you get your fingerprints done should be sufficient. I turned 18 in North Carolina and lived there from age 3 and half to age 18. The paragraph above seems to contradict its self. I'm a little confused, I wish there was a dummies guide to all this. (sigh)

Regarding your second question, I'm not sure what you don't understand or how it contradicts itself. It's saying to need to provide a police certificate for every country you lived in for 6 months or more since the age of 18. If that is only the USA then you only need the FBI certificate. If they want state police certificates, they will ask for them at a later date. If you are confused about "family members" that only means those who are eligible to be included in your application. In your case, that would be any dependant children you have.
