I was wondering if anyone can help with Financial Evaluation Form IMM5768? As I found this form hasn't been updated, Q9 shows year 2013,2012,2011, which suppose to be 2014,2013,2012! Did anyone
notice it? Shall I just change it myself?

You are using the old form. There is a new version available dated Nov 2015

It looks like they updated Nov.2015, but once I opened the form it shows an old version which was the same as last year. Do you have a new form? could you send it to me? Thank you so much!

Thank you all! I found the new form IMM5768 by using the searching tool! weird, on the front page it's the one old.

Sorry if this question has been discussed, I could not find the answer: does anybody know approximate statistics from the previous years on how soon the 5,000 quota can be filled? I mean, how long it might take for CIC to get 5,000 applications starting January 4th?

so for the IMM008 form, my parents have visited canada multiple times but are out of the country.

So if the option is greyed out, should we fill it by hand?

We have already filled out question 12 with details of last visits.

Additional Family Information Form (IMM 5406)

I have to fill one each for my dad and mom.
My dad is the primary applicant so do I put his name under Applicant on both the forms? Bit confused or one of the form should have my mom's name as applicant?

It took about a day and a half to reach the cap. It will most likely be reached on the first day this year.

Q5 :- Provide details on the back of the page if he or she is not co-signing the undertaking :- but the guide states Note: Provide details on page 2 of 4. So should I provide her details or not ?

Provincial allowance for an instruction or training program :- Where would I find this on my NOA
Social assistance from a province :- Where would I find this on my NOA
Amounts paid to you under the Employment Insurance Act other than special benefits :- My wife was on maternity during one of the eligible years. Would i add that amount here , or under total income line 150? From a previous post it showed that maternity is a special benefit so I am guessing it should not be listed here?

For the Fee payment, the sponsor is "1" written on the form, do we have to change it to "2" if we have a co-signer? i.e. $75*2=$150?

Also for the 3 consecutive taxation years the family size will be:

Sponsor + co-signer = 2

OR Sponsor + Cosigner + Mom + Dad + 1 dependent = 5

Please suggest. Thanks.
