#17 - An Option C printout is a document requested from Revenue Canada showing your source of income. There really is officially no such form, as I understand it, you call them and ask for it and they know what it is and send it to you.

#18 - You should include the letter from your employer in the US, but you also need to include any job offers in Canada or transfers back to Canada if that is how you will be employed when you return with your spouse.

#19 - Spouses are exempt from the LICO requirement, but you will want to include information regarding any savings or investments if you plan to use these to support yourselves until you are established in Canada.

Yes, spouses are exempt from meeting a Low Income Cut Off threshold. However, CIC can refuse a PR application under section 39 if they are not convinced that the person is willing or able to support themselves and that arrangements have been made for their support that do not involve reliance on welfare.

As you have not been in Canada, and are re-establishing yourself when your spouse is granted PR, you need to demonstrate how you intend to support yourselves. I think you should be able to ask for the Option C for the last year filed in Canada, and include your most recent return filed in the US.
