I was preparing for immigration since years, so I was asking each employer for the required form before I leave them.
Try your best to get the required form..
And my next step is to send my complete package to CIC..

By the way, when the first time they ask you to send the additional docs did your settlement fund already change? i mean the date of conversion already change? cause mine haven't change.
