A warning sign on view personal information disclosure is shown in my account now and if I click on it, I do not find any document or anything.

Does anybody have any knowledge or view?

It's the same for me. After clicking on it, I get '← Select a Record.' , but the whole page is empty and looks broken

I submitted my application in April and see the same thing..So it might not have anything to do with the submission date..Just a glitch I guess..
