The picture is probably zoomed, that's why it takes the whole size. Open with paint or some other app that allows you to edit (but don't edit). You should be able to see the real size.abdulqayyum said:i have taken following steps, but still have certain queries

Birth certificate is usually an identity document, so if you want to prove your identity to somebody, and not somebody else's who was born on a same day and was given same name, then yes it should include your surname.
