Yes this is a MUST form. You only need to fill up page 1 and 2. In page 1, you need to sign then on page 2 there are boxes to tick.
If you are not using any representative, you need to tick the last box and then sign.

hello Guys ...
have one question ---- i have submitted the SINP application.
At the time of submission i have attached both forms relevant ( like Schedule Background, Schedule 4: Economic class forms ) to me and spouse. But now in submitted application, only forms related to me are visible but spouse's forms are not there.

Actually I added both the forms ( mine and spouse ) in the same TAB.

So should i send it again to the SINP documents email ID. Can you please help me out.

I don't think it would create any problem. Or you can wait for them to ask for it. What is the status of your application anyway?
Is it in "In process" already?

As you are saying that we need to sign on both section A and B, but as per ethical-conduct-disclosure-and-declaration-form form , it is mentioned that Section A should be signed by

1. assisted by anyone
2. recruited by foreign recruiter

so can you please clarify on this.

but I saw many people were asked to send this form so I was curious to know about that.

According to SahiSh and according to the guide, we have to sign only Section B.

Hi Ann,

As far as I understand, since you have updated passport details in Express Entry, you have got the request to provide IMM5786 / SINP-100-1 form. Point here is, job seeker validation code is generally the last four digits of your passport number. I reckon the same might be the case with kksem.

I was thinking the same thing as I was expecting to hear more people reporting that they received nominations. On the other hand we have seen a steady rate of applications receiving "In process" statuses and the spreadsheet only has less than 15% of the total number of applicants, so it's a very good sign they are working, so hopefully this week we will get more positive news.

Some status has been in Process for more that 40 days without nomination. It is alarming!

We have a similar situation. My wife is the principal applicant and her NOC is 4011. Her current status is returned and they sent a letter asking for IELTS Results, Bank Statement and conduct code. At the end they also said, we encourage you to reapply.

The problem is that there is no chance to submit a new application. We don't know if they would consider an email with the missing documents.

Have you had any response for your case?

But earlier you said they did it by mistake because they put name and DOB of other person. You said your occupation is non regulated and they asked for license also. Please keep update what is the actual matter

Yes, they wrote the wrong data in a first letter but after my email response, they sent me another letter which said that NOC 4011 doesn't need license. However they asked for Bank Statement and IELTS results again. Additionally they said me that encourage me to reapply.
