The question about been refused immigrant/PR/visitor/refugee status is a different thing.
Having an APPLICATION rejected for, i.e, IELTS scores, or CAP been full (which was my case) is totally different.
The application for PR only was rejected, not your PR status itself.
Now, for example, if by any given reason your application for PR (via FSW or any other stream) was accepted and, at a later stage, you were refused, then, and only then, should you mention that on that form.
Cases such as misrepresentation, illness, background check or whatever the reason for CIC not accepting your request for a PR status, should be explained at the proper field on that form.
His application didn't pass the completeness check, but by no means that is the same as having a immigrant/PR/visitor/refugee or whatever status denied.
My generic application form (the one requested by SINP) has my UCI number because I was issued one, even though my application was returned to me because of the CAP limit.
UCI numbers are unique, and remain the same for each person through out their lifetime. Even if, for example, I have never applied for immigration, but only visited Canada as a tourist, I'd still have a UCI number because of my tourist visa. When filling out an application for immigration, and, subsequently all the forms, one should mention their UCI number, so that particular immigration file falls under your "main file".
