SGVO Requires Below Document:
Original good conduct certificate (or certificate of no criminal record) to be obtained from the
Bangladesh police authorities. A certificate is required for each family member, 18 years of age
or older, from the police authorities in all countries in which you have resided for at least six
months. There may be more than one type of police certificate available in each country. To find
out which one you need and how to apply for it, visit the CIC Web site:
http://www.cic.gc.ca/english/information/security/police-cert/index.asp. If you do not provide the
correct type of police certificate, there will be delays in processing your application
Proof of relationship to your relative in Canada and that your relative is currently residing
in Canada (statutory declaration alone is insufficient).
Proof that your relative is currently physically residing in Canada (eg. Current bank account
statements, tax returns) Passport, citizenship or permanent residence cards are not evidence of
residency in Canada
Provide completed Geneological Chart to demonstrate how you are related to sponsoring relative.
Detailed employment reference letters from your current and previous employers. These letters
should include a detailed job description and must clearly indicate the full name, title, business
address and contact numbers of the person signing.
Current bank statements and other evidence of funds and personal assets.
Documentation of who is offering a job and description of duties.
Education Diplomas and transcripts of marks.

IMM 5669 Schedule A Background / Declaration form, completed with up-to-date
information by you, your spouse/common-law partner, and each of your dependent
children who is 18 years of age or older. Fill the form on the CIC Web site then print it:

You must provide complete details of your personal history and addresses since the age
of 18. Do not leave any gaps. Failure to account for all time periods will delay the
processing of your application.
Pay the Right of Permanent Residence Fee (RPRF) for you (and your spouse/commonlaw
partner, if applicable). The instructions are attached
