My spouse received a package today with all our evidence and a letter request :
1)police certificate
2)imm5669
3) 4 passport-sized photos
4) Original passport

I'm wondering if he need to redo his medical check since they didn't request that on the letter.

Hello!

There is a spelling mistake of my wife's name on their record. Mistake is on their part, we submitted correctly spelled name on the application. When we came to know mistake is on their part, my lawyer emailed them 3 times for last 3 months duration. But they did not fix it, on the last letter we got from them they still spelled it wrong.

Do you know what would be the consequences if the name is not fixed?

How CIC/SVO can mis-spell your wife name ?

Because IMM 0008 form has barcode and CIC only scan that form and all the data go to their system within 5 seconds. Your Lawyer most probably wrongly spelled the name of your wife in IMM0008 form.

My medical was done in Oct 2013, so its already expired but i dont know why they didnt ask for that
