Could please clarify what is medical extn ? Does it mean your spouse will have to do medical again? I do not know you can contact passport specification or not. If you want just send an email and clearly tell that your decision was made 7th April but still you did not get your passport back. We received an envelope at our home address in Chiang Mai, Thailand. In the passport request letter :
1- They ask to provide an updated Thai police certificate, but it's also said that "If you have already submitted a police certificate which is still valid, you do not need to submit a new one". The certificate we provided with our application was issued on August 2013, so it's ok, right?
2- They say that Thai nationals MAY send their passport through the Canada Visa Application Center in Bangkok (VSF Global), but since we live in Chiang Mai, we'll have to send it there by post anyway... We'd rather send it directly to Singapore by registered mail... Do you think it's ok? Thai Post is very reliable, and we'd have a tracking number.
3- I'm not sure I understand correctly this warning : "Visas can only be issued to a maximum validity of 12 months from the date of your most recent Immigration Medical Examination (IME) assessment." My wife had her IME last August. Does it mean that the visa the will affix in her passport will only be valid until August 2014?? I thought the visa was always valid for one year. If not, perhaps we'll have to do the IME again...
Help would be appreciated. I emailed Singapore, but as we all know, they are not very responsive...

I would send the passport by registered mail or courier directly to Singapore
If they issue the visa and copr quickly you will only have until August to land in Canada... we had to redo part of the medical for an extension so therefore we have until 2 years from the original medical to land
