Sealed doctors letter

I have several questions , hope someone can help me with this.

My wife did medical test in NY and doctor gave her heavily sealed envelope with 5 of his stamps on the back of the envelope.
should I open it? or include with the rest of the documents as is? (sorry if its a silly question but one mistake can cost me dearly)


Also my wife is making me as a representative filling out form IMM5476

3-rd question of section "A" confused me:

If you already submitted your application:

Name of office where the application was submitted ...............
Location of office ..............

Since I'm sending this file with the rest of the application technically I've already submitted my application... or have I?

can someone help m out here, its too late my brain hurts :-\

Answer is no.You didnt already send in your app.So send the sealed envelope with the app but what is confusing me is the doctor gave you a sealed envelope,he should have only given you a green sheet with pictures attached with a bar code on it.The sealed envelope???Did you get the medical done by a certified immigration doctor?

I would open the envelope! There was no need for the doctor to seal it, it shouldn't have resuts in it, just a form with her picture and information for CIC to obtain the results from the medical processing centre.

You have to make sure you are sending in the right thing, so open it up!

Yes the doctor address i got from the canada immigration website. I was hesitant to open the letter because it is sealed and his stamps are right on the opening fold on it's back as if he was making sure this wouldnt be pre-opened. he told my wife to give it to sponsor.

Looks like Im gonna have to contact him, secretary doest know.

Oh so he told your wife to give it to you....well then like bobswife says i would open it too and take the form with her picture on it and include that with your app, thats how they check the results is from the barcode on the green sheet of paper.
