Thanks Gaurav....yes my wife has changed her last name after marriage infact only when we renewed the passport. She belongs to UP so i am thinking i would approach the local PS there but i dont think we can ask them to write both her former and current last name on the PCC document. That is what i find worrying.

Can you also confirm the WARSAW visa office address that you will be mailing the documents to?

PCC - if your wife has changed her only when passport was renewed and if she is having any other document proof of her earlier name with your current address, try contacting local PS and show them the mail from warsaw and ask if they could provide pcc with her former name............this could be possible only if u have any document evidence to show her former name with your current address.

Visa's remain valid even if new passport is issued... you just have to carry both the passports till the visa is valid...

I submitted my passports for Visa stamping on 12-May-2015.. Today i got a msg stating your visa appln with tracking id ******* has been forwarded to Visa office, Bangalore on 13-May-2015. Any idea how long it will take for visa stamping?

In my view, the change of name post marriage should reflect in the marriage certificate. If the marriage is registered with her maiden name, then it wasn't a nice move to change her name in the passport as all the other docs including marriage certificate bear her former name.

Generally, a PCC whether issued from PSK or local police station would have the same name as mentioned in the passport. You can verbally request the police officer to mention her former name but I am sure if it will work.
