I think you need to use their UCI number and their details (their UCI should be on the medical forms that you received)

Seems like NDVO works on alternate days. Monday was quiet, Tuesday came up with a storm of PPRs and now Wed is also quiet!

I have written a mail to NDVO and again raised a CSE with "most urgent" in the headlines and subject matter requesting them to issue my PPR and PR and COPR mentioning them i need to join a client of my current employer by May 2015 .I have been selected as an IT Consultant thru my company.Further with details of Application with medicals and company address in Canada

My employer is ready to give me a Intra-company transfer to Canada as my resume was selected by a client but still I am waiting for third line update and PPR...

Completed medicals on Nov-24-2014 and medicals uploaded on Nov 26-2014
No third line update till date (Waiting for third line for more than 4 months)
GCMS says medicals still in process but security and criminality check have been cleared in Jan 2015.

I paid it online thru visa card, do i have to mail or email the scanned/filled receipt ?
To which address should i send ?

I want to know, whether the Panel doctor will scan and upload the below mentioned documents to CIC or Visa Office or not. Is there any other form, which the panel doctor submits to visa office.


1. Medical Report (IMM 5419 E) form

2. CLIENT CONSENT AND DECLARATION (IMM 5743 E) form

Secondly,

I want to know, do we need to fill the Name, Date of Birth and IME number in the Medical Report form or the panel doctor will fill that.

The VO sends you separate medical forms for each applicant in your file and those forms have already been filled out by the VO. so you don't need to fill out anything. Just print out the forms and take them to the medical center alongside with a passport size photo for applicants (they will scan and return the photos to you if they have e-medical) and passports.

Yogi, if you have already sent so many CSE and mails, i dont think you can do anything more than wait. Rather ask your client for some extension so that you can join a month later in the project.

Yes you have to send the receipt by mail to the VO after filling the info on it.
Note that you will be charged about 30 $ if you are in Lebanon because you have to pay it in CAD. that what happened with me.
