When you complete the process. In receipt, fill your name, address, email, UCI no and mobile no with pen.

Good day. For my baby medical, please let me know as per below.

1) For baby, under 5 years old need to go for medical?
2) If yes, CIC will send medical form with IME number like our medical form to email?
3) For medical, what kind of medical check-up, need to do? My daughter is only two months old.

Before CIC send medical FORM to my email for my daughter, I am going to submit require documents (except my daughter medical FORM as this medical form come with IME number, so I must wait this form from CIC) for my daughter on coming 12-June-2015 as per below. Please check for me

1) Updated Generic Form (IMM0008ENU_2D) to add in my daughter particulars,
2) Updated Additional Family Information Form (IMM5406E) for me and my wife,
3) When pay CAD 150 for my daughter, I will use credit card. So I need to use IMM5620E?
4) Photocopy of my daughter passport ( can get on 11-June-2015)
5) Birth Certificate of my daughter (already done)
6) Passport photos 4 pcs

Any other documents I miss out? Please check for me. Thanks in advanced.

Hi.
I too had that question. You can submit passport between 9 & 4 pm . passport collection refers time to collect our stamped passports .

Hardly it take ten minutes if you are ready with documents and remember please do carry exact change for the charges in cash .

Hi Myat,
There won't be any Medical Examination for below 5 years old, they just do physical examination, but it's better to carry the discharge report and the immunization report... from my personal exp they just checked my son and asked some general questions that's it for my 8 months old son All the above documents are suffice... Your good to go All the best

Thanks for your info. Baby medical form is like our medical form including IME number? As my baby is now outside of singapore and IN THIS CASE, i need to bring my baby to singapore to check physical condition at singapore clinic at Nee Ann City clinic and she is only 2 months old. Dear Forum Members,

Need your suggestion urgently regarding my application.

Recently on June 02, 2015 I have got email from CIC saying I need to do paternity and maternity test. From the letter I realised I have mentioned my “Step sister” as my “Sister” mistakenly in my additional family information form. I have sent them a letter of explanation about the mistake and also said I am ready to do whatever test deems required by CIC. I have send them, her birth certification, her passport copy, my father's second marriage certificate copy.
I am very sad, everything looks like falling apart  I do not know if CIC accepts explanation letter at this point. I am already done with medical tests.
Now I need suggestion what should I do? Should I do the paternity and maternity immediately (they asked me to do that to prove that my sister is my sister since we have age gap of 24 years and they are not convinced, however that was a mistake from my end) or wait for reply from CIC based on my clarification sent to them?

Your suggestion will be very helpful. Since I am doing my application by myself so ca not get any lawyer advise at this point.

Hello all,

Congrats to all who have got ur PRs and now planning to land. I have a query which is not related to landing but related to the process that you all have followed before receiving ur DM/PPR request.

I had received medicals email on 15th May and got it done on 16th may. I made rprf payment also the very same day through CC. On 17th May I collected all documents: passport copies (front and back pages), medical forms signed and stamped by panel doctor, medical payment receipt, online RPRF receipt and a cover letter and scanned them all and emailed it to WVO. (Only scanned copies emailed, no hard copies sent by courier)

my TLU was done on 26th May, which confirmed that my medicals have been received.

Now, I come to know from many other forum members that I need to send ALL PAGES of my passport and not just the front and back page...it means that I need to send all blank pages as well as all past VISA Stampings done on my passport. IS THIS CORRECT? Do I need to resend all the documents with all pages of passport?

I am sure only you ppl in this forum can help me with the correct response as you have already gone through the entire process and have received your PRs.

Please advise so that I can take immediate action and resend all the documents through email if required.

Sorry ur going through this...Suggest u do the Paternity and Maternity tests as per CIC instructions to avoid delays.....It will not have a negative impact as long as u comply, don't worry....

Dude,

We from Delhi VO have received the email saying this->

"Please send photocopies of your updated/valid passports (only front and back Bio pages) for all . "

Whereas u from Warsaw VO would have received this->

"Please send a copy of all pages of the valid passport(s) for yourself and your accompanying family members..."

The instructions are quite clear..and u shud comply with ur VO-specific instructions to avoid any complications...U only have to scan and send, what's the problem in sending the blank pages?

In fact, I see a forum-mate already replied to ur query with specific instructions, please follow them exactly->

Background verification is MANDATORY for each and every applicant. It usually happens once you submit your PCC and is been carried out by third party. Though it is not clear when do they conduct it before of after MR, but its initiated only after your submit your PCC.
