First of all, this is a normal question to ask due to all that legal language obscurity. Anyway, this question doesn't annoy like the never-ending questions from thousands people again and again about how long it will take for the email to come after ITA.
I think that this new innovation would be most likely framed like this:
Do you have a sibling (brother or sister) who is a Canadian citizen or permanent resident, who is currently living in Canada and is 18 years of age or older?
Then there will be a button for "No" and "Yes".
If you click "Yes" you will get 15 additional points. Unfortunately, only 15 no matter if you have one or more siblings.
