Additional family information imm 5406
These two forms are required from your spouse and can be scanned
Or photocopies copiez.

Now my question is they require it from my spouse, then
Whose name will be put in the family information form
Applicant column and who will sign at the end of this form.

And for the form imm5669 who will sign at the end ofform me or my spouse?

Yes , as it indicates in documents checklist
I don't know if they will return your file , they may request you to send certified true copy of the degree
