After looking around, it seems like this is the document I apply for an open work permit: http://www.cic.gc.ca/english/information/applications/extend-worker.asp
In it asks for what employer I am going to be working for, but before that it asks what kind of permit, which I would select the open work permit. So not to sure what I would add under the employer part then.

If that is not the right document, can someone let me know? It has been a pain to find the right document so far.

As for the medical, I might end up doing it before we send the forms anyway.
The current processing times seems to be 11 months, so my medical should be fine IF they don't take longer.

Any advice would be appreciated,

I am also applying Inland (Common-Law) and have not been able to find the OWP application for applying within Canada.
This will be the first time that I will be seeking work in Canada (in a very small market...musician that just wants to play music in a live venue [bar/club] primarily),
so I don't have an existing (or previous) WP to extend/renew. I have also found IMM5410E, in the hopes that it is the correct application. The confusing part is that
I don't have an employment offer and will likely be working/playing `all over town'...someday. :D

I'll let you know what I find, in the hopes that it helps with your situation as well.

I am still very confused about the medical. Are you going to do yours before you send in your applications?

Based on the information that I've found for Inland applicants, it would seem to support waiting until at least Stage 1 approval, so...no.

Yes, that's the correct form. Write across the top of the form "OWP with inland sponsorship" and put N/A for employer and LMO details.

Current processing time for Stage 1 is 11 months. However, your medical must be valid when they issue your PR after stage 2, around 19 months.

I am cancelling my medical and will wait till they request it instead of doing it now and maybe again later.

I have only a few last questions that was answered before, I just want to be sure.
My visitors visa expire in February, and this whole process will take longer than that.

Since I am applying for the OWP with the sponsorship, I don't need to apply for an extension to my visitors visa right?
Do they receive your application, mark it off and let you know your good to stay for the period of stage 1 then?

The OWP will give you Implied Status when your visitor visa expires, so you do not have to apply for any extensions.

CIC will not send you any confirmation of Implied Status. If you are ever required to prove your status (very unlikely), you can use the OWP payment receipt and the delivery information showing the app was sent before your visitor status expired.
