For question # 5, "What is the highest level of education you have completed?", there is an option to check which level (primary, secondary, college, post grad, etc) and then a box next to it where you are supposed to fill in number of years completed. In that box do you write total number of years or just number of years completed in that level? For example, I did a 4 years doctorate program , so i checked off "Post Graduate Studies". In the box would i write 4 years (grad school only) or 20 years (total school years)?

For question #8, you are supposed to write down all the places/addresses you have resided. During my final year of grad school we did 3 month rotations in which i spent 3 months in japan, 3 months in oklahoma, and 3 months in Las vegas. Do I still have to include these in my application even though it was only a temporary 3 month stay?

Question 5: Enter the number of years for each level, e.g. "Post-graduate studies" ---> "Number of years completed" 4.

Question 8: Enter all the information about places you lived, as a rule I entered all addresses for stays longer than 1 month.

You have to write the highest level of education completed and then the accumulated number of years dating back from the most recent, fill in the number in each box that correspods to the level of academic degree.

One more question...for question #11 "are you currently living in canada?, if yes, since when?" If i lived in Canada my whole life except for a 4 year period in which i was a student in the US, would i write the date of when i moved back to canada after those 4 years or do i write "since birth"?
