Questions about initial application for spousal class

Hey, I have a bunch of questions if someone has the time to help..

1. The document checklist states forms 5406 and 5669 are required for my spouse and "each family member over 18 year's of age." Does this only include her (no kids) or also her brothers, mother and father (whom aren't being sponsored )?

2. Related, would the background and medical check also only be required by her?

3. How are testimonials from friends (proof of our relationship) made? Just a few statements to that effect or is something more formal required?

4. Since my spouse's English is not very good and I am assisting with all of her paperwork, would she need to fill in the representative form?

5. Finally, the doctor said he will do the medical examination after our visa office receives the paperwork and send us instructions, but my checklist states that I should be sending it with the original application. Which is corrected?

1. As far as I know, 5406 asks for details about her parents, siblings, kids as well as spouse. 5669 is for her plus all dependable children 18+ years of age or older. So if your spouse has no kids than she just needs to fill it out once for herself.

2. If she is the sponsored applicant than yes she has to get the medicals done.

3. Honestly, the ones I got were all in different formats with some very formal and others not so much, but I left them as is. I don't want the VO to think I made it up by have them all look like in the exact same format. However, in my case they are optional so I didn't worry to much about the format.

4. Not sure about this one. Sorry!

5. Which office are you using? Did you chose from the list specified? Here is the link to find a panel physician in your spouses area http://www.cic.gc.ca/dmp-md/medical.aspx.

to answer question 4: you don't have to fill out the representative form as it is "normal" to assist your spouse. However, if you want all information to go to you ( the first part, CIC sends information to the sponsor, for stage 2, it goes to the applicant ONLY), then fill out the representative form, like this all information will go to you and you can discuss with your spouse and make sure everything is clear.

For the medicals : it HAS to be done before, some doctors are a bit confused, make sure you specify that you are applying through spousal sponsorship (for other immigrations classes, liked skilled workers etc, the VO gives instructions to do the medicals when they are ready to process the file, but for the spouse class, the medicals have to be done BEFORE the application is sent, so depending on your area, the panel physician might think that you have to wait for instructions from the VO, but that's not the case for spousal.).

Wow, thanks noshi11 and Sweden! Those answers came in a lot sooner then I expected.
The doctor is the biggest hassle since he is sure he has to wait for personalized forms but everywhere else says I need the medical to be included.
I have printed out IMM5727 and IMM5419 and will accompany my wife to the hospital. Are there any other forms I may need?

A designated medical doctor should a) know what forms he needs; b) have them in his office.

This doctor doesn't sound very professional. Do you have another option?

The forms that you are quoting are the one that the physician should have, not you, so there is no need for these ones. The only one that you need to bring is in your application package, and from what I understand from the recent changes, it is not needed anymore. The only things you should bring are pictures ( depending on the doctor, it can be 2, 3 or 4, but better be safe than sorry and bring 4), and money to pay for the exams.... but I agree with Amikety - if you have another option, try and call another doctor because this one doesn't seem very informed.

I'm applying to the Hong Kong office since I'm in Taiwan. There are only two options for doctors and one is on the opposite side of the city.
We'll go Monday morning and I'll bring the forms. It may just be my poor Chinese ability. I'll update the thread afterwards.

My doctor did ask me to bring Form A filled out (the one that lists my name, address, etc) but told me if I didn't bring it, they could give it to me. It was only to save me time at the office - get me in and out faster.

Besides the green and white page they gave me, I never saw the rest of the forms.

Very interesting experience! Said none of those papers were needed and started some new ones. After getting all of the tests completed,the doctor began filling in one of the same forms I had originally brought! :eek:
See well, it's all finished and now almost ready to send in the application.

Hopefully at the end of the exam the doctor gave you a signed copy of the IMM1017 form, which you then need to include in your application.
