    I'm applying to the Hong Kong office since I'm in Taiwan. There are only two options for doctors and one is on the opposite side of the city. We'll go Monday morning and I'll bring the forms. It may just be my poor Chinese ability. I'll update the thread afterwards. Thanks again!
