Questions about Form CIT 0002

I am filling the for CIT 0002 for my citizenship application.
I have some doubts about a couple of sections.
In particular about section 9:
I had PR in Sept. 2017, so i have to provide my status before the PR.
I have been on different work permits from June 2012, there were no gaps between the permits, but I have absolutely no track of them.
Would it be ok if I just put one entry that goes from June 2012 till the date I received permanent residency?
If this is not ok. Do you know how can I get the dates of the work permits?

You have to provide your status, not the different documents that extended this same status.
One entry and you're good.
