question regarding pr application under live-in caregiver

hello there. i am about to submit my pr application soon under the live-in caregiver program. i have few questions in mind, hope you can help me :)
1. there is a question on imm 5002 form schedule 1 number 6. have you or your family members been refused a temporary /permanent visa to canada or other countries?
-i am confused whether i will answer yes to this question. two years ago i was asked by my employer to apply for a visitor/tourist visa to the US coz i need to accompany his 75yr old father. i undergone an interview and got denied coz i didn't meet some qualifications. the officer told me that i can still reapply in the future. Would it affect my application if i will state yes to this question? do i need to provide supporting letter/document regarding the refusal? is it a major offense?

2. iam a single applicant with no dependents. on imm 5002 it was written on page 2 and 3 that i need to to write all my family members living inside and outside of canada. it is stated that i must include spouse and children. i don't know what to write because i am a single applicant with no children/spouse. can i just put n/a (not applicable) or do i need to put the name of my parents and siblings?

This is based on what I did on my application.

1. I also have been denied by US when I applied for a tourist visa. But I cannot remember the exact date of my application coz my old passport was misplaced during flood back home. I just noted an estimation. I clicked yes and gave the reason. If you still have your passport with the stamped refusal, photocopy it and include that with your application.

2. Your parents and siblings are not considered dependents. Yes, you just put N/A.

thanks plussy for the reply! btw, there is no refusal stamp on my passport when they returned it to me after the interview. they just gave me a letter. have you already received your pr visa? :) did you also answer yes to question have you been refused entry to or ordered to leave canada or any other country? thank you so much!!

Photocopy the letter and include it in your application. Make sure it has the date of your application and place on the box provided 'Please see attached sheet'.

Not yet. Just submitted mine last month.

I only clicked YES to 'refused a temporary or permanent resident visa'. All other questions on No. 6, I said NO.

I am under the live caregiver and will be applying for my PR this July.I realized I did not get a copy of my contract before I mailed to CIC.I asked my employer if he has a copy of our contract and he did not have a copy too.I was told by CIC to provide a written letter why I can't provide the original contract when the time comes I submit my application for PR.Will this be a problem in my application?

Hi Chuchay,

In my opinion, you're just fine. As long as you submit a copy of your contract w/o signature (as you stated in other threads) and an honest and thorough explanation as to why you have no copies of the signed contract.

Will you be applying with dependents or a single applicant?

Make sure now that you make copies of all documents on your OWP/PR application. :)

I have a question, i'll be applying for my PR this july under 3900, my friend is asking me how did i compute my 3900 hours, well, i just add the hours from my ROE from my previous employers and just add the hours that i'm making now with my current employer. Am i right? Or i have to call someone from cic to know if i really complted 3900 hours? And how about those vacation leave with pay, can i add those in my 3900 hours? Thanks! :D

I am supposed to submit my PR application in July but due to I have to work straight for a month because of summer vacation,my application got delayed.I called CIC sometime in June for the forms needed for PR but since I am submitting my application this september should I not be using the new forms?CIC mailed me in June the old forms.Is it still acceptable?I dont want to go through the whole thing again thus delaying my application some more days again..Anyone who can answer my query thank you so much ;D

thanks.What I did was I printed the very same contract and had it signed again by me and my employer with the current date and I attached a letter why I did not provide the original copy.Will that be ok?

Re apps: I suggest that you try to complete the new application form which is found in this link.

I don't know if the previous one is still applicable. But in this way, you can prevent any future queries from CIC as to why you did not use the new forms. :)

Yes. You must submit all documents from current and previous employers.

by the way..I submitted the old form for PR/OWP.I called CIC and they said the old forms are still accepted til Feb next year.(mailed mine in Sept 2011)
