Question regarding appendix a, passport request

so this Appendix A is a confirmation of important details ( for flight ) and confirmation of important data like the mailing address , ( to where to mail the visa) . notice the things that ask on the form its not the same on IMM 5669, according to my idea but I didn't have this one yet am still in waiting

It is not the same as IMM5669 or the document checklist. I believe the form is specific to the Manila office, as I've not seen any other VO send it out.

Thanks Canuck-in-UK, that clears things up. Very odd that they would name this doc the same as the checklist, but it appears it's only something that will be sent once the process has (hopefully) evolved to the decision point. Still don't understand why they wouldn't just ask for these details in the original forms.

Well, CIC isn't known for their clarity.

I believe Manila gives it to all PR applicants, not just Family Class. Economic PR packages don't have an "Appendix A".

Just needlessly adding more time to the process too Hope I find the energy someday to write Immigration Canada about this whole shoddy process.

Where can i get this form? Its actually a relief to see this kind of form that says Appendix A! but where can i find it?thanx

I also tried to call my VO but they are not entertaining questions over the phone,being routed to an IVR.

Reply to the original email you received saying that the Appendix A was not attached. It's not a form that you can find on CIC's website; Manila emails it to you.

I think this might be it.
At the bottom it says "a copy of this document checklist"
Did you send this with your application? (along with the general checklist)

No one on here can provide you with the form. You must wait for the VO to resend it to you.

We tried to email our VO to ask for that form, but they told us to wait for their responce within 25 days, but in our case we where given 45 days to resend the forms

Hello! just want to ask you guys if any one knows what is completed APPENDIX A(see attached). My wife received her PR last Sep.17 and now received an email saying (Us) the dependents requires
1. Original passport
2. 2 pcs passport sized id photos
3. completed appendix A(see attached)

The problem is there is no attachment, we tried emailing them again but until now no response yet, anyone experience like this before? or have an idea what is appendix A?

Hi guys, just for clarification on the Appendix A form. For us (dependents) are we going to fill out the child 1,2,3,4 and 5 columns right?
My mother is the sponsor and we are the dependents. (me and 4 sisters).
