It looks the same as the other AORs except without your numbers. Have you called them since to ask for a number? Even a number you can refer to when you call?

If they still aren't showing, another thing you can try is downloading a new blank form and filling in all of the information again. Validate it before you close it. Some of the forms are temperamental. It is a pain, but it would be worse to send the application in without the barcodes!
