It depends on your province:
Ontario has a 3 month grace period. You can apply at any time but your coverage won't kick in until 3 months.
Passport and COPR is fine but you might as well wait for your PR Card.

Other provinces have varying rules.

The application process is summarized here:

You will need to carry 3 types of documents (summarized in the link above) and fill in an application form.

1) Proof of Citizenship or OHIP Eligible Status
I took COPR
2) Proof of Residency
I took valid Ontario Driver's license
3) Support of Identity
I took foreign passport

The coverage will activate after 3 months.
