For each job declared in the Application for Selection Certificate: •Work certificate or attestation Each work certificate or attestation must contain the following elements:
–The exact period of your employment at the company
–The number of hours worked per week
–The positions that you have held and the period during which you held each position
–The principal tasks and duties of each position
–The signature of your immediate supervisor or the company's personnel representative along with his name written in block letters
–The date of the signature.

oks lang po yan. Pag ok na yung application mo at malapit ka na sa Passport request eh pwede mo na ipa renew passport mo. Bsta pag may Medical request na kayo eh pa renew nyo na agad passport nyo ok. Goodluck GOd bless

salamat po sa reply..tanong po ulit, dun po sa form A-0520-AA (application for Selection Certificate) question no. 5...leave as blank lang po ba ung sagot since same address naman namin ung mga anak namin? or need pa po magnote like "same as applicant's"?

Hi to all! Please help me with my documents. My documents was returned early this morning because I lack the A-0520-AF and A-1520-AF. Does this forms have an english version? Because I can't fill it out since I don't really understand french.what shoild I do?

Do you mind posting the actual letter you received from MICC?

I am not sure about that. But to me, it does not matter since your employment history is attested by your manager and supported by your SS/tax contributions. If you have nothing to hide, that should not be a problem.
