This refers to your application for permanent residence in Canada. Please return this letter with the following requested documents:

- Generic Application Form for Canada – IMM 008
- Schedule A – Background / Declaration – IMM 5669
- Additional Family Information – IMM 5406
- Supplementary Information – Your travels – IMM 5562
- New Lebanese Police Certificate
- New original family list

All documents can be downloaded from the following site:
http://www.cic.gc.ca/english/immigrate/quebec/apply-application.asp
Please complete and then print the forms. All forms must be signed. Please note that you need to update the above documents even if they have already been submitted. Please ensure that you do not leave any unexplained periods of time when you list your personal history.

Documents bearing other languages must be accompanied by original certified translations into English or French. All police certificates must be originals.

Please gather all requirements and submit ALL requested documents at the same time, in a single package, by mail to the Embassy of Canada in Beirut at the address below :

Just a question regarding "new orginal family list"...where do you get that from? Also, should you fill out the documents in french or english? Do you think it matters?

The family list can be brought from where he gets the ID, it is not an issue but of course, his parents have to apply for it at the "saraya", it takes 2 days. I think in my case I should get the one that includes me and my wife now. The documents can be filled either in English or French, it doesn't matter either way...

I believe that you still have the chance before 5/6/2013 to get your visa without undergoing a new medical exam. (they can extend your medical 3 months up to 15 June 2013).
Hurry up as fast as you can. They are finalizing your file. Help them by updating all the informations as fast as possible. Thus, they can issue your visa before 5 of June, else, you may be obliged to redo a new medical.
I just have a question: have you paid the visa fees 1040 CAD??

my friend paid also same amount, he didnt get visa yet, but they might not ask you for new medical.

So the medical exam is about $200? and the visa is $490?

your comments are very helpful and suppprtive. I believe that all QSW in Beirut are frustrated.
As, no visas. No medicals .... Do you think they will work hard from now on? If the trends will remain the same, I feel that we will wait forever.

Hey Moon, i'm not very anxious to go anymore, it's been 3 years and 3 months since the Federal part, it is bollox... i am tired about the whole issue, and Cyprus isn't that bad apart from the economical part of course. anyway, i might just stay here and enjoy the sun and the beach.

You can enjoy the sun for days and months but what about your future? Don't lose hope, sooner or later you will get. As we all say, no news is good news. Your file is somewhere among all the other files. Hopefully soon I'll get some updates from my mp and let you know how things are going. Meanwhile keep gaining more experience cause it will help a lot in hiring you fast when you move. Study the Québec market as well. I've been there and be sure that you're gonna love it. I'm talking about Québec side since they're all frenchies there and very friendly

I got a reply from the mp office today in Ottawa after sending an inquiry yesterday, it was really fast, I guess they're working hard at the embassy. The last update I received was 8 months ago informing me that my file is in queue for review and will be seen by the VO on April 22 and I should be hearing from cic max by mid May. Today I got the same news that my file will be seen by the VO on April 22, I guess it means good news since I got the same update. I've been waiting for 29 months so hopefully the end is near... Fingers crossed!!!
