- Generic Application Form for Canada – IMM 008
- Schedule A – Background / Declaration – IMM 5669
- Additional Family Information – IMM 5406
- Supplementary Information – Your travels – IMM 5562
- New Lebanese Police Certificate
- New original family list
All documents can be downloaded from the following site:
http://www.cic.gc.ca/english/immigrate/quebec/apply-application.asp
Please complete and then print the forms. All forms must be signed. Please note that you need to update the above documents even if they have already been submitted. Please ensure that you do not leave any unexplained periods of time when you list your personal history.
Documents bearing other languages must be accompanied by original certified translations into English or French. All police certificates must be originals.
Please gather all requirements and submit ALL requested documents at the same time, in a single package, by mail to the Embassy of Canada in Beirut at the address below :

Just a question regarding "new orginal family list"...where do you get that from? Also, should you fill out the documents in french or english? Do you think it matters?
