I received a quality assurance check documents and I need to provide provincial health claims form. I was in Ontario for 2 years and now I am in Alberta. For the Ontario health claims form from OHIP, I saw this link which doesnt have any standard form to fill out , I just need to write a letter. (http://www.health.gov.on.ca/en/public/programs/ohip/phi_access/default.aspx)
There is one more link that I saw: http://www.forms.ssb.gov.on.ca/mbs/ssb/forms/ssbforms.nsf/GetFileAttach/014-4890-84E~1/$File/4890-84E.pdf which gives a PDF form to fill out but the mailing address is always set to ON and I cannot change it.
My question is:
1) To get a Ontario health claims summary, do I just send out a letter providing name/mailing address etc and would they just mail me back the document ?
OR
2) Do I fill up the PDF (shown in second link above) but the mailing address cannot be set to anything other than ON. and I am in AB now.
Any help is greatly appreciated.
