Hi there,

I am seeing my doctor next week for a regular check up and I wanted to ask him for the letter stating that I am expecting. I am 21 weeks along now and figure it's a good point to get the confirmation letter and could also include a copy of our ultrasound picture. :)


My question is; does CIC require a specific format for the doctors note? Can it just be handwritten and signed? Or should I type something up and ask him to sign?

I just don't want to send it in and have it not be admissible for something simple like that.

Any advice would be great!

As it's not required, it doesn't really matter how it's done.
 
It can be hand written on the doctor's letter head and signatures, more than enough. We also did the same and sent the ultrasound scans as well. These scans were conducted on different centers so it had multiple reports of different med centers and it dint matter.
 
I found out that I was expecting after my meds were done and all I did was email them with my due date. They didn't request and ultrasound or a doctor's note. They only requested his birth certificate after he was born. But as always, every VO is different.
 
Ohhh ok, thanks a lot guys!! I figure the rate CIC is working at we'll be updating when little dude is born most likely!
